Specific Responsibilities:

T his is Nurse-Family Partnership (NFP) Senior Community Health Nursing Supervisor position responsible to direct and support nursing program specialists in their efforts to engage, support and serve first-time mothers enrolled in the Nurse-Family Partnership Program, and meet program goals to improve pregnancy outcomes, child health and development and parental life course for families in Palm Beach County. Work is performed under the direct supervision of a Community Health Nursing Consultant. This position requires independent judgment, initiative, applying general knowledge of business practices and skill in applying instructions to accomplish various job functions.  This position spends the majority of the time engaged in supervisory responsibilities. This includes communicating, motivating, training, and evaluating employees in addition to planning and directing employees’ work. The incumbent has the authority to hire, transfer, suspend, lay off, recall, promote, discharge, assign, reward, or discipline subordinate employees or effectively recommend such action. The incumbent will also be required to develop performance expectations, meet with staff regularly to discuss performance, complete performance reviews, maintain vacation and work schedules, ensuring proper coverage, and complete timesheets. Adherence to all established Florida Department of Health Safety Procedures and Practices while in the workplace and in the community.

This position is designated as a sensitive position and requires maintaining confidential information in accordance with the Florida Department of Health (FDOH) Palm Beach County Health Department (CHD) and Children’s Services Council (CSC) Information Security Policy, Protocols and Procedures. Confidential data sets: Has access to include Clinical Information, Health Management System (HMS)-Employee Activities Record (EARS), Florida Shots, Healthy Beginnings Data System (HBDS) and Nurse-Family Partnership Data System (FLO) for the purpose of entering and reviewing client data to view, update or release client information and uses an electronic medical record database. 

Assigns client caseloads and monitors staffs ability to build and manage caseloads. Provide  face to face and not face to face services to program participants to meet grant requirements.  Oversees and ensures timely and accurate data input and utilization of web-based reports and other reports available through Healthy Beginnings and NFP-NSO. Works as a team player and creates strong working relationships to accomplish goals of the Agency, the Healthy Beginnings System of Care and the NFP National Service Office (NSO). Participates in trainings and meetings as required by the DOH, NFP and the Children’s Services Council of Palm Beach County. Ensures that staff attend NFP and CSC mandatory education sessions.

Assesses and supports the professional development of staff by identifying nurse’s strengths and areas for professional growth resulting in the continuous refinement of skills. Coaches and provides feedback utilizing strength-based approaches and interventions. Implements regular reflective practice supervision and conducts field visits, both with the frequency recommended by NFP-NSO. Operates with fidelity to the Model Elements of the NFP program and adherence to the NFP Agency Implementation Agreement and monitors program outcomes. Functions as a resource for staff, providing both clinical and technical assistance including but not limited to assessments, case management activities and electronic documentation.

Provides programmatic reports and input to the Community Healthy Nursing Consultant for strategic planning of the program. Attends meeting in the absence of the consultant. Assists in planning and conducting team meetings, team education, client case conferences, and staff meetings. Orients new staff. Participates in continuous quality improvement of the program. Participates in Healthy Beginnings System of Care work groups. Completes data analysis of client satisfaction surveys. Develops written procedures, protocols, processes, worksheets and reporting tools as assigned. Assists with the development of the NFP budget and reviews expenditures for accuracy and reimbursement.

Qualifications

Licen sure/registration/certification requirements:

• This occupation requires licensure as a Registered Professional Nurse in accordance with Chapter 464, Florida Statutes, and in accordance with rule 64B9, Florida Administrative code.

Other job-related requirements for this position : 

• Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in special needs or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man- made or natural.
• Valid Driver’s License.
• Out of state and/or out-of-county travel may be required for mandatory trainings and continued education.
• This position works primarily in an office setting and involves sitting for long periods of time, and the moving, lifting, pushing and carrying objects up to 25 lbs., multiple times throughout the day, and occasionally working in a community setting providing home visits and involves getting in and out of a vehicle, walking up and down stairs.

(C) This position may be required to work before and/or beyond the normal core business hours of 8:00 AM to 5:00 PM and on weekends due to program needs.
