Job Title: Registered Nurse (RN)

Location: Brandley House and Hall House

Position Type: 24 hours a week/ Saturday and Sunday from 7am to 7pm.

Key Responsibilities:

• Demonstrates the ability to obtain patient history perform an age-specific physical psychosocial and spiritual assessment and initiates appropriate referral intervention and collaboration between the patient family Interdisciplinary team (IDT) physician and partnering facility (as applicable) following policy and practice guidelines.
• The RN will identify patient/family goals develop a Plan of Care with patient family IDT attending and physician partnering facility and evaluate progress towards goals. The Plan of Care will be updated according to policy/guidelines and as needs change (RN performs as case manager).
• The RN will provide collaboration and care coordination between the patient family Interdisciplinary team (IDT) physician and partnering facility (as applicable) following policy and practice guidelines.
• The RN will use excellent critical thinking judgment and ethical principles for problem solving positive and effective collaboration and follow-through in all patient and caregiver clinical situations.

Requirements:

• Education and/or Experience: Associates degree (A.A.) or equivalent from two-year college or technical school; Minimum of or six months to one year related experience and/or training; or equivalent combination of education and experience.
• Licensure:
  • Valid Florida license as a Registered Nurse
  • Valid Florida State Drivers License
• Continuing Education: As required for licensure.
• Certifications: Valid CPR certification from the American Red Cross or American Heart Association
• Minimum of 1 year of current active nursing experience; with experience in caring for terminally ill patients or those with life threatening illnesses preferred; prefer one (1) year experience in an acute or post acute care setting.
• Post Offer Employment Testing (POET) or Fit for Duty examination (as necessary) with Capable result required.
